Instructions
- Complete a nursing assessment. The assessment includes information taken directly from the patient, significant others, a physical assessment, labs, vital signs, other diagnostic tests and in consultation with other health care providers.
- Check the NANDA-approved nursing diagnosis list. Find one or several that fits your assessment.
- Write the diagnosis in the approved, standardized manner. An example of a NANDA diagnosis related to risk is, "Risk for Impaired Liver Function." Another example of a NANDA diagnosis is, "Pain, chronic."
- Re-assess the patient. The NANDA nursing diagnosis may either be resolved or changed as the condition of the patient changes.
